A phone can be in Spanish with the region set to the United States. If the app infers units from the language, that person will see kilometers for the rest of their working life. The region is the setting to read, and even then it's worth making it changeable inside.
The number pad changes its separator
The decimal key follows the device's regional setting, so the same field will receive commas and periods depending on who is typing. Accept both, store one, and have the confirmation repeat the figure with its unit so whoever typed it can say it's wrong.
The unit preference travels with the account, not the device
If the choice lives only on the phone, the same person sees one scale in the app and another on the web, and has to pick again after switching handsets. Stored on the account, the preference follows them everywhere and survives a new phone.
A reading taken offline keeps the unit it was taken in
A reading taken in a warehouse with no signal gets uploaded hours later. If somebody changes the preference in the meantime, that record can't be reinterpreted: it was written down with its unit and it stays that way. What changes is how it's shown, never what was measured.
